To avoid squishing surrounding areas you can ‘paint’ over them with a quick mask (in same dialog box) and they’ll actually be unaffected by the bloat tool! May even be able to adjust *how* unaffected (in order to not give too sharp a contrast if needed)… I always mask off the surroundings if I’m making fairly large changes, (I would’ve in his instance, certainly), but in a pinch or if you don’t think it’s noticeable it’s not always necessary… hope that helps!

The only problem is, when you enlarged the general area, it sort of squished her arm/side over a bit, which looks weird. But, I understand these tutorials are rushed, so… :)
